RESEARCH CONSENT FORM
PROJECT PURPOSE:
Outcomes of this study will be used to improve instructional effectiveness and understand what activities are significant and meaningful to students in DSGN 130/430/ARCH 525 from Spring Semester 2015.

RESEARCH ACTIVITIES:
Participants in this study may participate in any or all of the following:
SURVEY – A basic inquiry into the significance and meaning of design thinking and creativity, and team work and small group decision-making are related to the Design Thinking and Innovation coursework.
FOLLOWUP INTERVIEW – A 15-20 minute interview to understand the personal significance of the course in a personal narrative. This session will be audio recorded and take place on the phone, videoconference, or face to face.
SMALL GROUP DISCUSSION and IDEATION SESSION – A 60 minute 4-7 person session to discuss and ideate new ideas for the course. This session will be audio recorded. This part of the research study may be limited due to geographic location. If you would like to participate from a distant location you would need to use skype or similar conferencing technology.
RESEARCH DURATION AND SCOPE:
You will be one of approximately 35 individuals participating in the research project. The project is expected to last 8 weeks. Your participation may be in part or whole and will vary in type and duration. You will be contacted a minimum of 24 hours in advance prior to initiating any research activity with you. You may choose what activities to participate and you are not under any obligation to participate in part or whole.
RISKS OF PARTICIPATION:
There are no known risks associated with this project that are greater than those ordinarily encountered in daily life.
BENIFITS OF PARTICIPATION:
Benefits of this study include improved instruction in Design Thinking and Innovation courses.
CONSTITUENTS RIGHTS:
I understand that my participation is voluntary; that there is no penalty for refusal to participate, and that I am free to withdraw my consent and participation in this project at any time, without penalty.
PRIVACY and CONFIDENTIALITY:
Subjects will have anonymous participation and the results of their activities will be kept private. Research documents and records generated by anonymous participates will be stored in a locked office and a password protected computer. Researchers will be responsible for data collection and access to the records. All records will be destroyed at the end of the project.
AUDIO RECORDINGS:
Interviews and small group sessions will be recorded in electronic audio format. Audio recordings will only be used to generate transcripts and subsequent analysis. Recordings will only be used by the researcher and destroyed after the transcripts are made.
If a participant withdraws from the study the audio recordings will be destroyed and not used in the project.
